Please be advised that when you register for a class online or over the phone, you are agreeing to the following:
I hereby attest and acknowledge that I have been informed of the following pertaining to the coronavirus:
• COVID-19 is an extremely contagious virus that spreads easily through person-to-person contact. Federal authorities and the State of Connecticut recommend social distancing to prevent the spread of COVID-19. Contracting COVID-19 can lead to severe illness, personal injury, permanent disability, and death. Use of Groton parks & Recreation (“GPR”) facilities or participation in GPR programs could increase the risk of you or your child contracting COVID-19.
• GPR and its staff undertakes every effort to keep our facility clean and disinfected; however as with any public facility, we cannot guarantee that you will be 100% safe from airborne illnesses such as COVID-19 or colds and flu while using GPR facilities or participating in its programs.
• Each participant must perform a daily symptom assessment for any symptoms of the COVID-19 CDC-defined symptoms, including cough, shortness of breath, or any two of the following symptoms: fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, new loss of taste or smell. If any symptoms are present, do not participate.
• People who are 65 years and older, people of any age who have serious underlying medical conditions, or are at higher risk for severe illness from COVID-19 are recommended to stay at home. A list of medical conditions associated with a higher risk for severe illness from COVID-19 can be found in CDC’s guidance. Individuals and families should consult their healthcare provider to determine whether they have medical conditions that place them at risk.
• Staff and children living in households with individuals who are 65 years and older OR have higher risk for severe illnesses from COVID-19 are recommended to stay home.

In consideration of my participation or that of my above named individual in activities conducted and/or sponsored by the Town of Groton Parks and Recreation Department, I acknowledge that such activities are potentially hazardous and pose a risk of injuries that can be significant and that I assume such risks. I agree to release, hold harmless and waive any claims that might arise against the Town of Groton Parks and Recreation Department, the Town of Groton, and their officials, representatives, agents and employees on account of any and all injuries and claims of injury to person while participating in and/or traveling to and/or from said activities.
